Termination/Transfer/Less than 80 Checklist (rev 9.2021) Employees who have terminated, transferred, or are at jeopardy of falling below 80 paid hours in a month. Please note that this form is interactive, but it must be downloaded first.
Military Reservist Called to Active Duty (rev 9.2021) Employees who will be on a long term deployment and must elect to keep or drop benefits
Direct Payment Authorization Deployed military who pay the premium from a checking account
State Auditor's Office Vendor Payment Form Direct deposit from a reimbursement account
ONLINE ORIENTATION - for Employees
An online orientation is available for your employees to use if your agency/entity human resources office has chosen to utilize. Be aware this is not intended to replace the function of HR. Any employee who is provided the training link and agency key, is responsible for completing and obtaining the new hire checklist at the end. The checklist must be provided to HR and forwarded to EGI before any benefit enrollment will be honored. Benefits Training for Human Resource personnel is available from EGI quarterly. The training presentation is developed from the contents of the Benefit Specialist Handbook. Whether you are a new HR employee or you are an existing HR employee and want a refresher, please feel free to email email@example.com to sign up for a scheduled training. If your agency has a new HR staff member, please reach out to Karyn for the appropriate material/forms while waiting for the next training session. Training presentation is presented via Google Hangouts and is from 8:30am - 12:30pm.
